What does the SPOT Foundation do?

We help improve the quality of life of people with musculoskeletal conditions who, for economic reasons, cannot afford the cost of their medical treatment.

How do I apply for aid?

To request help, you must complete the Request for Assistance form and attach the required documents.

What documents are required to send with the application?
  • Copy of the patient's birth certificate
  • Photo of the patient (full body)
  • Copy of identification (driver's license/passport); if a minor, identification of the father, mother or legal guardian
  • Water or electricity bill
  • Proof of income
  • Proof of expenses
  • Copy of Income Tax Return
  • Medical order/therapy indicating diagnosis and specifications of the equipment or treatment
  • Certification of medical plan coverage
  • Certification of medical plan denial
What kind of service can I request?

How far in advance should I submit the application?

Whenever possible, we should receive the request one month in advance.

How long does the evaluation process take?

We try to complete the evaluation process as quickly as possible. Everything depends on receiving the application and all required documents, as well as the corresponding quotes.

Do you make payments in the applicant's name?

No. Payments are not made in the name of the applicant or their representative. Payments are made directly to the supplier, hospital, or other entity.

I don't have health insurance, can I apply for aid?

Yes. We specifically want to support people who do not have health insurance.

I have health insurance, can I apply for aid?

Yes. Medical plans sometimes have limitations on their coverage.
If you have a medical plan and the equipment or treatment you need is not covered, you must submit a DENIAL CERTIFICATION so that we can evaluate your request.

In which towns do they offer aid?

Any resident of Puerto Rico can request our help, regardless of the town where they live.

How old do I have to be to apply for assistance?

We offer help to people of all ages, from newborns to adults over 100 years old.

My condition is not being treated by an orthopedic specialist. Can another healthcare professional provide the medical referral?

Yes. Any healthcare professional who treats musculoskeletal conditions can refer you (physiatrists, radiologists, physical therapists, occupational therapists, rheumatologists, among others).

How can I donate?

Is my donation tax-deductible?

Yes. Our organization is registered as a 501(c)(3) non-profit entity, so your donations are tax-deductible.

How are donations used?

They are directly allocated to patient aid, including medical equipment, surgeries, therapies, and other essential services.

Can I donate a specific amount or make recurring donations?

Yes. You can select both the amount and frequency of your donation, either as a one-time gift or monthly.

Can I donate medical equipment or services?

Yes, you can donate medical equipment and services. Your contribution is very valuable and directly helps to strengthen the aid we offer and the impact of the foundation.

Do I need prior experience?

No, you do not need previous experience to be a foundation volunteer. The most
important thing is your willingness to help, commitment, and desire to contribute to our mission.
